Information you need to know before treatment
Our office hours to schedule appointments are 8 a.m. to 5 p.m., Monday through Thursday, and Friday 8 a.m. to 12 p.m. Scheduling an appointment is an easy process. To schedule an appointment, simply call our office at (502) 326-8588 and press option #1. When scheduling an appointment you will need the following information:
- Patient Name
- Date of Birth
- Patient Phone Number
- Address
- Reason for Visit
- Insurance Plan
At the time of the appointment you are required to bring proper identification and a current copy of your insurance card, unless paying out of pocket for services.

Patients that are interested in being contacted for an appt
NOTE: If you have an HMO plan you must have a written referral sent to our office at least 48 hours prior to your scheduled appointment time. You can bring the referral to our office or have your primary care physician fax the referral to our office. The fax number is (502) 326-8589.
